The mission of The North Carolina Department of Health and Human Services (NCDHHS) is to ensure the health, safety and well-being of all North Carolinians. It addresses the human service needs of vulnerable populations like the mentally ill, deaf, blind and developmentally disabled, and helps poor North Carolinians achieve economic independence.

Abstract

This State Partnership project will address the disproportionate chronic disease burden of minority and American Indian (AI) populations across the state. North Carolina Department of Health and Human Services (NCDHHS) will focus on encouraging and helping community-based organizations and AI Tribes engage in effective chronic disease interventions. To this end, NCDHHS will assess their chronic disease prevention and intervention capacity through formal assessments and focus groups discussions about health disparities. The project will also improve provider efficacy through web-based cultural competence trainings. Finally, NCDHHS will provide technical assistance to participating organizations. The intended outcomes for this project include increasing access to health services for minority and American Indian populations, increasing the number of minorities in policy-making positions, and improving the overall minority health in North Carolina. In order to determine if such outcomes are reached, partner organizations will regularly report program and participant data, such as the number of people taking assessments and trainings per year, number of hours spent with the clients, and the number of individuals newly enrolled in health insurance programs.

  • Strengthened leadership and coordination to leverage resources and enhance effectiveness and efficiency of individual and collective efforts (including, but not limited to, research and data)
