The Social Security Administration published the revised Form SSA-89 (12-2020) (Authorization for the Social Security Administration (SSA) To Release Social Security Number (SSN) Verification) and Form SSA-89-SP (Spanish). You can access the revised forms in the CBSV Documents submenu located on the CBSV menu.
Please note:
- You have until September 30, 2021 to transition to the 12-2020 forms.
- SSA has NOT changed the signature policy for CBSV users, therefore these forms require a wet signature.
- We are not able to provide a fillable PDF for the SSA-89-SP.

Due to COVID-19, the Social Security Administration implemented a work-from-home policy. As a result, our Office of Finance - Reimbursable Agreements changed how we receive and process CBSV payments. Please plan to send us timely payments to avoid any disruption to service. Include your CBSV agreement number when you submit your payment using one of these methods:
Pay.gov
Pay.gov is the fastest way to credit your CBSV account. You can request this method by emailing DRAC/IABT at . After submitting your request, you will receive an email from pay.gov indicating that the eBill is ready for payment. Unlike credit cards, this method is suitable for payments over $25,000.
Credit card
Use this credit card form //www.ssa.gov/forms/ssa-1414.pdf and include the CBSV agreement number in the “This payment is for” field. Email the completed form to and allow 1-2 business days for processing. Please note that the Department of Treasury limits credit card charges to $24,999.99.
Paper checks
Because of the new work from home policy, this method is more likely to result in a disruption in service. If you use a check, you MUST email us with the carrier name and tracking number to so that we may track the payment. Be sure to include the agreement number in the memo field. Allow 10 business days for either standard or “priority overnight service” via USPS. Allow 7 business days for overnight FedEx or UPS service.

You must have an Employer Identification Number (EIN) to enroll in CBSV. If you need an EIN, contact the IRS.
About CBSV
SSA is currently developing a new electronic Consent Based Social Security Number Verification (eCBSV) system which will allow permitted entities to accept electronic consents. Please see the following link for more information on eCBSV.
The Freedom of Information Act (FOIA), the Privacy Act at 5 U.S.C. § 552a (b), section 1106 of the Social Security Act, codified at 42 U.S.C. § 1306, and SSA regulation at 20 C.F.R. § 401.100 provide legal authority for disclosure of SSN verifications from SSA records.
CBSV is typically used by companies that provide banking and mortgage services, process credit checks, provide background checks, satisfy licensing requirements, etc.
With the consent of the SSN holder, CBSV can verify if the SSN holder’s name, date of birth, and SSN match SSA’s records. CBSV returns a match verification of “yes” or “no.” If our records show that the SSN holder is deceased, CBSV returns a death indicator. CBSV does not verify an individual’s identity.
Fees include a one-time (non-refundable) initial enrollment fee of $5,000, and a fee per-SSN verification transaction. The transaction fee is presently $1.00 per verification request. The CBSV transaction fee can change at any time.

Note:
CBSV does not verify identity, citizenship, employment eligibility, nor does it interface with the Department of Homeland Security (DHS) verification system. CBSV verifications do not satisfy DHS’s I-9 requirements.
